If Sisu has a single conceptual frame, it is Peter Attia’s. The “marginal decade” that Sisu is built to defend against is his term. The “Strongest Decade” is our inverse of it. The math we run with members during the Sisu Assessment, working backward from the activity they want to be able to do at 80 to the cardiovascular and structural reserve they need today, is the longevity framework Attia formalized in Outlive and on The Drive. Members who have read his book or listened to his podcast will recognize the language. Members who have not are about to.

Key contribution

Attia’s central contribution is the reframing of medicine itself. He calls the dominant model “Medicine 2.0” and argues that it is built around treating disease once it has appeared. His “Medicine 3.0” framework moves the intervention upstream by decades, focusing on the trajectory of an individual rather than the threshold of a diagnosis. The implication is direct. The work to live a strong eighth and ninth decade does not start when symptoms appear. It starts forty years earlier, with measurable physical capacities trained against a target.

Two specific Attia constructs underwrite Sisu’s programming.

The first is the Centenarian Decathlon. The idea is simple. Make a list of the ten physical activities you want to be able to do in your last decade. Hike with grandchildren. Lift a 30-pound carry-on into an overhead bin. Get up off the floor without using your hands. Each item has measurable demands on cardiovascular reserve, strength, balance, and flexibility. Working backward from those demands, accounting for age-related decline, gives you a training target right now. This is the math the Sisu Assessment runs.

The second is the marginal decade, the last ten years of most people’s lives, lived poorly because the cardiovascular and structural reserve to do anything else was never built. Attia’s argument is that the marginal decade is not inevitable. It is what happens when the upstream work was skipped. Sisu’s entire programming model exists to do that upstream work for adults who still have time to build the reserve.

Attia is also a consistent advocate for VO2 max as the strongest single predictor of all-cause mortality, for resistance training as co-equal with cardiovascular work, and for stability and balance training as non-negotiable past 50.

How Sisu applies this work

Three elements of Sisu’s framework are explicitly Attia-derived.

  1. Activity-anchored goal setting. The Sisu Assessment opens with the question “what do you want to be able to do in your last decade?” That is the Centenarian Decathlon translated into our intake conversation.
  2. Working the math backward. The MET-based calculations we walk through with members, comfort multipliers, decline buffers, and current-VO2-max gap, are the practical version of the framework Attia has explained at length on The Drive.
  3. Both engines, neither optional. Sisu treats cardiovascular and resistance training as co-equal pillars of the longevity intervention, not headline plus accessory. That position is consistent with the human mortality data Attia has surfaced repeatedly.
